New guidelines on HRT and weight loss medication

We would like to inform you about recent guidance from the British Menopause Society (BMS) concerning the use of GLP-1 receptor agonists, such as semaglutide (Ozempic, Wegovy) and tirzepatide (Mounjaro) in conjunction with Hormonal Replacement Therapy, Oral Oestrogen and Progesterone.

Progesterone is prescribed to all women with a uterus, taking any form of oestrogen.  One of the main functions of progesterone is to protect the uterus lining from getting too thick, as well as relieving other perimenopausal, menopausal and post-menopausal symptoms.

Semaglutide (Ozempic, Wegovy) and tirzepatide (Mounjaro) work by delaying gastric emptying and have common gastrointestinal side effects. It’s not yet clear how these medications impact the effectiveness of co-prescribed oral hormone treatments, like progesterone used in HRT. However, there is evidence that they can reduce how well progesterone is absorbed.

Therefore, the British Menopause Society has advised on a pragmatic approach to adjusting progesterone dosing with different HRT preparations:
