Can you fast while using weight loss medications?
Technically, it is possible to fast while using GLP-1 medications like Mounjaro, but it is not generally recommended by our clinical and health coaching teams.
Mounjaro works by mimicking hormones that slow gastric emptying and signal fullness to the brain. Most clients find their food intake naturally reduces by around 30%. When you add strict fasting windows to this already reduced appetite, it becomes incredibly difficult to consume the essential nutrients your body needs to stay healthy and feel energised as you progress towards your weight loss goals.
Is it safe to take Mounjaro while fasting?
Fasting on Mounjaro introduces several risks that can stall your progress and negatively impact your health:
- Muscle loss: Without sufficient protein intake spread across 3 or more daily meals, your body may burn more lean tissue, including muscle and bone as energy.
- Nutrient gaps: Significantly reducing your eating window or skipping meals makes consuming enough vitamins and minerals challenging. This can lead to nutrient deficiencies, and over time can manifest as low energy, hair thinning and frequent illness.
When working towards weight loss using Mounjaro, creating healthy routines and habits around regular, balanced meals, exercise and other positive lifestyle changes supports weight maintenance in the longer term.
Choosing the right fasting schedule with Mounjaro
If you prefer a structured eating pattern, we recommend a gentle time-restricted eating (TRE) approach rather than more restrictive fasting.
Some research suggests benefits from reducing the daily eating window. For many people, this means eating their final meal earlier in the evening and extending the overnight fast.
- A 10-12 hour eating window: For example, eating breakfast at 7 am and finishing dinner between 5 and 7 pm, supports natural circadian rhythm and has been associated with improvements in cardiometabolic health, sleep quality and energy levels.
- Avoid extreme fasts: Methods like “One Meal a Day” (OMAD) or 24-hour fasts are not recommended while on Mounjaro, as they make it nearly impossible to eat enough protein to protect your lean tissue and are not sustainable in the longer term.
Exercise while fasting on Mounjaro
Exercise, particularly resistance or strength exercise is a crucial element to include alongside weight loss with Mounjaro. Adding this to your routine supports muscle maintenance as weight loss progresses and supports a healthy metabolism. However, exercising in a fasted state while can sometimes be counterproductive. With a lower calorie intake it’s important to listen to your body, and if you experience dizziness, nausea, or a lack of power during your workouts you might like to include a small balanced snack before exercise to fuel your activity.
Hydration: A non-negotiable
Hydration is even more critical if you are fasting. Mounjaro can sometimes cause side effects such as nausea or diarrhoea, which increase the risk of dehydration. Since we get a significant portion of our daily water from food, fasting windows mean you must be extra diligent with your water intake throughout the day. A daily water intake of two to three litres is recommended.
What to eat: three balanced meals per day
To support healthy weight loss while maximising nutrients and preserving muscle, aim to include at least three balanced meals each day. Where possible, structure meals as follows:
- Protein, quarter of the plate:
○ Lean meat, fish, eggs, Greek yoghurt, cottage cheese, tofu, tempeh,
seitan or edamame
○ Aim for around 20 to 30 grams of protein per meal.
- Carbohydrates, quarter of the plate:
○ Choose wholegrain options where possible, such as wholegrain rice, wholewheat pasta, quinoa, sweet potato, wholemeal bread or wholegrain crackers.
- Vegetables, salad or fruit, half of the plate:
○ Such as fruit at breakfast, broccoli, peppers, peas, courgette or spinach. Aim to include a wide variety, to benefit from the different nutrients they contain.
- A small, thumb-sized serving of healthy fats if not included elsewhere:
○ Such as nuts, seeds, avocado and olive oil

Mounjaro can make fasting feel easy because normal hunger is suppressed, however maintaining a fasting regime without medication can be challenging. Instead, we recommend focusing on building a consistent routine which supports a healthy relationship with food. Eating regular, balanced meals while on Mounjaro helps establish strong foundations for maintaining your weight when you reach your goal and beyond.

Can I do intermittent fasting with Mounjaro? It is possible, but we advise caution. Prioritising protein and nutrient density is much more important for your long-term success than sticking to short eating windows.
